Description

The Texas Tech University Health Sciences Center at El Paso (“TTUHSC EP”), a part of the Texas Tech University System, seeks proposals from qualified consulting firms to conduct a comprehensive needs assessment and develop a strategic plan for establishing a Diabetes Patient Navigation Program serving patients receiving medical care in El Paso, TX and the Borderplex region. The selected contractor will work collaboratively with TTUHSC El Paso leadership, Clinical Operations, clinical departments, institutional programs, patients, and community stakeholders to evaluate existing services, identify gaps in care coordination, and develop a comprehensive, sustainable, and scalable model for a centralized diabetes patient navigation program. The proposed program is intended to streamline the patient journey by improving coordination among medical providers, specialty clinics, pharmacies, diagnostic services, diabetes education programs, financial assistance and community resources. The planning process will identify opportunities to reduce barriers to care, improve patient access and continuity of care, reduce unnecessary duplication of services, and establish a framework for future program implementation. The initial program is anticipated to be piloted at the Texas Tech Physicians of El Paso's Alberta Avenue clinics. The contractor will assess the feasibility of this phased approach and develop recommendations to guide future implementation. This engagement is a planning and program-development project. The contractor will not be responsible for implementing or operating the navigation program unless separately authorized through a subsequent agreement.
