Description
The Wisconsin Department of Health Services (DHS), Division of Quality Services (DQA) is seeking proposals for Surveyor Services and intends to use the results of this process to award a Contract. Surveyors are needed to initiate an investigation of complaints for Long-Term Care (LTC) and Non-Long-Term Care (NLTC) facilities and recertifications. LTCs are inclusive of, but not limited to, Intermediated Care for Individuals with Intellectual Disabilities (ICF/IID) facilities. NLTCs are inclusive of, but not limited to, hospitals and End Stage Renal Disease facilities. This solicitation is divided into two (2) Lots and will result in up to three (3) separate contracts for each Lot. Vendors may submit a proposal for either of the two (2) Lots, or for both Lots. The Vendor must ensure that proposal submissions clearly identify the Lot number for which the proposal is being submitted. For purposes of this RFP, each Lot is identified as follows: Lot 1 – Surveyor Services for Long-Term Care (LTC) Facilities Lot 2 – Surveyor Services for Non-Long-Term Care (NLTC) Facilities
