The Ramsey County Aging & Disability Services Division, within the Social Services Department, would like to contract with agencies to provide case management services for Ramsey County residents under the following programs: Alternative Care, Brain Injury Waiver, Community Access for Disability Inclusion Waiver, Community Alternative Care Waiver, Developmental Disability Waiver, Elderly Waiver, Essential Community Supports Rule 185 Case Management, and Vulnerable Adults/Developmental Disability Targeted Case Management.
