Grandview Blvd. � Waukesha, WI 53188 to procure a AssurePoint Standard Service Agreement for the following GE Healthcare system: GE PET Discovery MI-Digital Ready, System ID 301594DR710 and its DQA Imaging Phantom component ASSUREPOINT STANDARD SERVICE AGREEMENT INCLUDES: � 30 min ILINQ response time � System and tube coverage for up to 3,000 patients; � FE coverage Mon-Fri, 8AM-9PM; � 4 hrs. on-site response time; � iCenter Software Application � 30 min InSite response time; � InSite/Tech Phone Support � PM coverage Mon-Fri, 8AM-9PM; � Repair (OEM) parts next day 10:30AM; LST-PET � Software and quality updates; � TiP answer line; TiP-Ed online (TV) subscription; and � 97% uptime commitment. � GE Healthcare Factory-Trained/Certified Engineers/Servicing Personnel PERIOD OF PERFORMANCE: This will be a Base Year Plus Three Option Periods Firm Fixed Price Procurement.
