** 10 September 2026** RFP HT941026R2001 Amendment 0005 This amendment is issued to notify interested parties of the following updates to the solicitation and its associated attachments: � Solicitation Updates: Clarified instructions for submittal of previously completed past performance questionnaires either directly to listed Contract Specialist and Contracting Officer or included with the proposal submission. Clarified language regarding transition plan instructions to �execute a timely and effective phase-in period within 90 days of the contract start date in order to meet PWS requirements by the full-performance base period of performance start date.� � Attachment 1 (Performance Work Statement): Para. 1.6.5.1. has been revised to remove the Exhibit A- Physical Security Audit Matrix as it is no longer required. The physical security criteria and facility inspection standards are established under DoD Instruction 5200.08, DoD Manual 5200.08-R incorporated into section 6.3. References to MHS Personal Computer Hardware Requirements and MHS Standard Notebook Hardware dtd. September 7, 2000 have been removed. Para 4.3 has been updated with contractor-furnished workstation hardware and software minimum requirements. � Attachment 5 (Incumbent Information and PHIRMAC Data): The Puget Sound Total Enrollment Table and Hawaii Total Enrollment Table has been added in addition to the Combined Puget Sound and Hawaii Total Enrollment Table The closing date for the receipt of offers has changed; please see page six (6) of the solicitation for the revised deadline. Please be advised that a Government Accountability Office (GAO) protest is currently in progress under Docket Number B-424743.1, with an expected resolution date of November 23, 2026.
